Waiver of Liability: In consideration of my child/ren’s participation in ISBR Academy, I, as a parent/legal guardian of the child/ren named above, hereby agree to release, discharge, waive and hold harmless Islamic Society of Basking Ridge (“ISBR”), and its employees, agents, teachers, volunteers, trustees, officers and members (collectively with ISBR, the “ISBR Parties”) for any incident, actions or conduct resulting in personal injury, accident, or illness to any child/ren named above. I acknowledge and agree that none of the ISBR Parties is responsible for my child/ren after the conclusion of classes should I be late in picking up my child/ren. I expressly agree that this Waiver of Liability is intended to be as broad as is permitted by the laws of the State of New Jersey. If any portion hereof is held legally invalid, I agree that the remaining provisions shall, notwithstanding, continue in full force and effect. I have carefully read this Waiver and understand that it is a full waiver of any and all claims that may be made against, and any liability that may be incurred by any of the ISBR Parties.
Medical & Other Emergency Procedures: As a parent/guardian, I acknowledge that ISBR Academy will follow the medical/emergency procedures set forth below while our child/ren are students: 1. If a student suffers a non-emergency injury, such as an ordinary cut or bruise, a staff member will administer first aid to the student, and attempt to contact the student’s parents. 2. In the case of non-medical emergencies, a staff member will contact fire and/or police for emergency service, as appropriate. 3. In the case of medical emergencies involving any student, we consent to ISBR Academy to: (a) arrange to transport the student(s) to emergency medical care by public emergency vehicle or parent/guardian vehicle and we agree to pay any expenses for emergency care; and (b) make all possible attempts to contact the student’s parents.
